Community Consultation 2021

Country SA PHN is undertaking this survey to gain an understanding of your experiences in accessing the health system in South Australia. 

We want to hear the story of your experiences and what matters to you.  Your story might include: 

• Your experiences of finding and accessing the health care professionals you need; 
• How easy it is for you to attend services; 
• Are there waiting times for care you need in your area; and 
• If you have out-of-pocket costs for the services you need.

Your stories will help guide Country SA PHN in our commissioning work and in collaboration with our health system partners, strive to meet the needs you have identified.

We would like you to answer each question before moving to the next.   If you need to change a previous response, you are able to go “back” and make desired changes.
