    Local Health Clusters are seeking to engage with country South Australian communities through the survey below.
    The Local Health Clusters would like to draw on your practical experiences after being discharged from hospital or consulting with specialists to determine whether the communication between your health professionals serves country South Australians as well as it could, or whether there is a need for improvements.

     Through your practical experiences we would like to better understand the adequacy of:

    •  the communication between General Practitioners and Surgeons pre and post-surgery
    • the transition to local health services post-operatively

     We would also like to understand how these systems impact on clients’ post-operative care.

     The Local Health Clusters are a series of local community associations who meet to consider local issues and provide views and opinions to Country SA PHN.

     The results of this short survey will form the basis for Local Health Clusters to advocate to facilitate improvements should they be required.

     This is your chance to have your say about the quality of care you and your community is receiving and make suggestions for the future.

    As a member of a Local Health Cluster Country SA PHN is undertaking this survey to gain insight into your experiences of accessing health care in South Australia. Your responses will support us to further understand how country South Australians access health care and assist our work in building a collaborative and responsive health care system. 

    You can provide multiple examples and as a guide, one example should take no more than a few minutes.  All responses are confidential.

    Information from this survey will used by Country SA PHN as part of ongoing identification and assessment of health and service needs, and activity planning for your region.

    Country SA PHN (CSAPHN) recognises and respects the importance of protecting personal information and the individual right to privacy. CSAPHN takes its lawful obligations under the Privacy Act seriously, and will ensure the application of the Australian Privacy Principles. Any individual, organisation or other entity that provides information to CSAPHN is protected according to these principles.

    In response to the Aged Care Royal Commission, the Australian Government are funding Primary Health Networks (PHNs) to establish and maintain a network of Care Finders for older people in the community.

    The Care Finder purpose is to provide navigation support to help older people to understand and access aged care and connect with other relevant supports in the community.

    Country SA PHN are currently exploring needs and services related to care navigation for all older people in the country SA region.

    This has included finding out from key stakeholders the difficulties and challenges they see in navigating and access to services.      

    We would be keen to gain your perspective of what is going on the ground in your region.
