    Country SA PHN is undertaking this survey through your Local Health Cluster to gain an
    understanding of you and your community’s access to health services, as well as activities
    or programmes which support overall health and wellness.

    Your knowledge and participation will help guide Country SA PHN in our commissioning
    work and, in collaboration with our health system partners, strive to meet the needs identified. 

    The survey is structured in two parts. The first section is about what helps or prevents you
    and your community from accessing things which improve your health and wellbeing.
    The second section is about you and your community’s access to health professionals. 

    This section can be answered multiple times. 

    We would like you to answer each question before moving to the next and if you need to change
    a previous response, you can go “back” and make desired changes.

    Country SA PHN is undertaking this survey to gain insight into your experiences of accessing the health system in South Australia. You will be able to provide feedback on your experiences of finding health care professionals, ease of attending services, waiting times, and out-of-pocket costs for services. Your responses will help us to understand the ease at which country South Australians access health care and will also help to shape the direction of future surveys.

    We are interested in your experiences and you can provide as many examples as you like. You are also able to provide answers on behalf of a family member or friend, e.g. your child’s paediatrician appointments, or a friend’s visit to the optometrist, providing you can answer the details of their experience.

    As a guide, one example should take no more than a few minutes and at the end you will be given the option of providing another example or submitting. If you choose to provide further examples your current responses will be saved, and the survey will restart. This process will repeat, until you no longer wish to provide further examples.

    Your responses will be kept completely confidential and there will be no identifying information available on your answers.
